WELCOME TO CONSCIOUS CARE SUPPORT

  • Conscious Care Support is a specialised NDIS service provider that delivers outreach support services to individuals who are living with a psychosocial disability/mental ill health in Victoria/Metropolian Melbourne. You can view our service areas here.


  • Our services are specifically tailored towards individuals who live with a psychosocial disability/mental ill health and require specialised support. 


  • We have supported more than 1000 people in our community. 


  • We have qualified and experienced Outreach Mental Health Support Workers who are passionate about helping individuals living with a psychosocial disability/mental ill health meet their goals and fulfil their aspirations. 


  • Our Outreach Mental Health Support Workers work directly with participants as well as their care team to ensure effective service delivery. 


  • Our services are based on a social model of recovery. This means that we are passionate about helping individuals improve their psychosocial health through support services that encourage social participation and interaction.


  • Conscious Care Support delivers support services at the highest level with a personal touch. We bring empathy, compassion, and understanding to our services. 


  • We are a grass roots organisation with over 90 employees. This means that everyone at Conscious Care Support provides support work and we all work in the community to help individuals work toward their goals.

Why Choose Conscious Care Support?

We understand the need for person-centred and specialised mental health support in the community. This is why Conscious Care Support recruits support workers with a specialised degree in a mental health-related field. 


We feel this qualification, background and experience in mental health is essential when helping individuals living with a psychosocial disability/mental ill health work toward their goals.


We ensure quality in our support delivery by allowing participants of the NDIS to have a consistent, reliable, and ongoing qualified Outreach Mental Health Support Worker.


Conscious Care Support is a grass-roots organisation. This means that everyone at our organisation provides support work to help individuals living with a psychosocial disability work toward their goals.

We understand that it can be difficult to engage in the community all by yourself. Our Outreach Mental Health Support Workers can help support you to attend events, classes or groups in your local community. 


Examples could include attending a yoga class, a social event or party, or other support groups.
