We understand that engaging with some service providers can be a long and complicated process. Here at Conscious Care Support, we have made the process as simple and as easy for you as possible. This is how it works:

Complete the referral form above, submit an online enquiry or call us on (03) 8797 3499.

Our team will respond to your referral request to discuss our services and how we can help you.

We will organise a meet and greet at a time and place of your convenience. This is where you will meet your Outreach Mental Health Support Worker and discuss your goals, aspirations, and what you would like to get out of our services.

If you were happy with the meet and greet and wish to proceed with Conscious Care Support, our support services will start.
